Environments for ageing and dementia design assessment tool (EADDAT) – Tier 2 Self-Certification

The environments for ageing and dementia design assessment tool (EADDAT) allows you to assess your environment using the latest research and guidance. With tier 2 you can choose to undertake a self-certification using the included checklist within the pack. By completing the checklist you will know if your building meets the 'tier 2 dementia supportive' criteria. For buildings that meet the criteria, you can choose to promote your achievement through self-certification.

A self-certification includes:

• Certificate
• Window decal
• Digital decal 

To purchase the self-certification, you will need to upload a copy of your completed checklist.

You will receive the digital decal upon purchase, and the certificate and window decal will be posted out within 5 working days.
