DSDC has formulated a toolkit for use in a systematic approach when assessing a person with dementia for delirium. Determining if someone with dementia also has a delirium is essential as it can be a sign of an acute illness requiring prompt actions. Some conditions may be life-threatening or have the potential to become life-threatening when not actioned. It can also lead to unnecessary hospitalisation or administering inappropriate medication.

 This easy-to-use toolkit offers actions which can be undertaken quickly to try and alleviate some symptoms or causes of the delirium. It also takes into consideration hypo-active delirium. Not all individuals providing care are able to interpret observations, therefore this guidance has been included with actions indicated.

This toolkit is not a prescription for treatment, but it is for guidance and can be used to collect information for referral purposes (i.e. CPN and/or GP).  It should be used in conjunction with knowledge of the individuals baseline behaviours, presentation, observations as well as clinical skills and judgement.
